Certificate of Attendance/ADAP Form

  • CERTIFICATE OF ATTENDANCE FOR LEARNER’S PERMIT
  • To obtain a certificate of enrollment

    1. Pick up a Certificate of Attendance from outside the front office.
    2. Fill out Part A only and return to the box outside the front office.
    3. Forms received by noon each day will be available for pick up the next school day 3:00 – 3:30  in the front office.  Please remember forms are only good for 30 days.

 

  • CERTIFICATE OF ATTENDANCE & ADAP FORM FOR DRIVER’S PERMIT

To obtain a certificate of enrollment:

Pick up a Certificate of Attendance from outside the front office.

    1. Fill out Part A only and return to the box outside the front office.
    2. Forms received by noon each day will be available for pick up the next school day 3:00 – 3:30  in the front office.  Please remember forms are only good for 30 days. 

       To obtain an ADAP form:

  1.  You can either take the ADAP course online for free and print your certificate

                                      OR

 You must have taken PE/Health & Fitness at MHS and have passed the course (in this case you can request an ADAP form by emailing Mrs. Howard @stacey.howard@mcssga.org) and pick up your ADAP Certificate when you pick up your Certificate of Attendance.
